Unsafe driving caused a collision between two southern California fire trucks responding to the same fire that injured 15 people after one of the trucks headed into a restaurant, according to an investigation by the California Highway Patrol.

The investigation found fault with the driver of the Alhambra fire truck for the April 16, 2014, crash. However, no charges will be filed.
