A Bradley County (Tenn.) Road Department employee has been terminated after he struck two colleagues with a county dump truck while backing up, reports the Cleveland Banner.

James Droke was fired Nov. 5, following a department internal investigation into the Aug. 3 accident that led to the amputation of both legs of Debra Norton Jenkins. Tedd Dobbs was also struck but suffered no serious injuries.

An investigation by the Tennessee Occupational Safety and Health Administration found that the department committed a serious violation by failing to provide employees with safety training around operating a motor vehicle, especially with "the recognized hazards associated with small dump trucks or similar medium-duty trucks."

Droke said he didn't remember if he had applied the parking gear and said the truck was in gear when it was parked in front of the department garage.
