Unsafe driving caused a collision between two southern California fire trucks responding to the same fire that injured 15 people after one of the trucks headed into a restaurant, according to an investigation by the California Highway Patrol.

The investigation found fault with the driver of the Alhambra fire truck for the April 16, 2014, crash. However, no charges will be filed. The two engines were responding with lights and sirens activated to a fire at a Monterey Park home when they collided in an intersection, reports the Los Angeles Times.

Following the collision, the Monterey Park truck struck a pedestrian and a street pole before ramming into the restaurant, Lu Dumpling House.
